Family Handbook
Our goal at the Children’s Center is to provide a secure, loving, educational environment for all children! We are committed to guiding every child in our care in developing their full potential Family Handbook – In our Family Handbook you will find information regarding the policies and best practices we implement at Children’s Center of Damascus.
The Emergency Form is a required document per MSDE Office of Child Care for each child. The form must be filled out, signed and dated and provided to the Center upon enrollment. Emergency Forms must be updated annually.

Health Inventory
A medical examination is a requirement for all children in care per MSDE Office of Child Care. A completed Health Inventory is required upon enrollment.
A record of immunizations meeting the immunization guidelines set by the Maryland Department of Health and Mental Hygiene for each child is required by the MSDE Office of Child Care. It is important to provide the Center with copies of any ongoing immunizations received during the enrollment period with the Center.

Medical Administration Authorization Form
In the event that a child would need medication (oral or topical) during their time at the Center, a Medication Administration Authorization Form should be completed and signed by both the physician and parent or guardian.
Children with allergies are required to have an Allergy Action Form completed and updated annually. The Allergy Action Plan should be completed and signed by both the physician and the parent or guardian along with a Medication Administration Authorization Form for any medications that may be needed.

Asthma Action Plan
Children with asthma, requiring the use of medication for the condition, are required to have an Asthma Action Plan competed and updated annually. The Asthma Action Plan should be completed by both the physician and the parent and accompanied by a Medication Administration Authorization Form for any medications needed.
